I acknowledge that by completing this form and by my child attending sessions at Rebel 4orce Cheer, I understand and accept that participation in this program or activity involves inherent risks of physical injury. I voluntarily assume full responsibility for any and all injuries, damages, or losses, regardless of severity, that my child may sustain as a result of participation.
I further agree to forever waive, release, and discharge any and all claims that I or my child may have now or in the future arising out of or in connection with my child’s participation in this program or activity against Rebel 4orce, including its officials, agents, volunteers, and employees. I do hereby fully and permanently release Rebel 4orce from any and all liability for injuries, damages, or losses that I or my child may incur, now or at any time in the future, in connection with or in any way related to this program or activity.
By signing this form, I confirm that I have read, fully understand, and voluntarily agree to the above information, including the warning of risk, assumption of risk, and perpetual waiver and release of all claims.
I give permission for my child to be photographed and/or video recorded during classes, activities, and events conducted by Rebel 4orce Cheer & Gymnastics
I understand that these images and recordings may be used in promotional materials, including but not limited to social media, websites, flyers, and newsletters.
I acknowledge that by attending Rebel 4orce Cheer & Dance programs, I am granting permission for my child to appear in photos and video content captured during these sessions.
I understand that no compensation will be provided for the use of these materials, and that Rebel 4orce Cheer & Dance will use them respectfully and appropriately.
