2026 Individual Act Entry Form (due by July 11th by 11:59pm)

Rules:

If you register after July 9th by 11:59pm, you will be seeded at the beginning and owe an additional 50% of the entry fee. Registration will close at 7:30 a.m. on Friday, July 24, 2026. If any items are missing at the close of registration, the competitor(s) will be ineligible to compete in the Individual Competition. No refunds will be issued.

The maximum number of individual competitors will be 70. If a division doesn’t fill, other divisions will be allowed additional entries in the following rotation: doubles, swivel-open, freestyle jump-open, swivel-180, team jump, intermediate freestyle jump and swivel pairs. All tournament participants will be filled on a first-come, first-served basis.

Seeding is as follows: The top 3 competitors from the prior year’s Midwest Region Tournament shall be seeded in reverse order from their final placement. Doubles couples must remain an entity. A change in a partner will be considered a new couple. Team jump will be exempt from the same participant rule. All other entries will be drawn from a hat and seeded late to early. The starting order will be posted immediately following tournament registration.

The Individual and Show Act Competition Rules will be followed. A copy of the rules can be found at www.usawaterski.org. Plaques will be awarded to 1st, 2nd, & 3rd place winners for each event.
