12. Safety Boat: Certified Driver & Certified Safety Rider Name & Phone Number:Each team must provide a driver and safety rider for Friday and for another team’s show on Saturday or Sunday. If using different people for each day, please indicate which day each driver/rider is assigned. Submit names and cell numbers. The safety boat schedule will be shared closer to the tournament date.

13. LIABILITY RELEASE

The undersigned club and its individual members understand their responsibility to exercise necessary precautions and act in a safe manner at all times. They agree to release USA Water Ski and Wake Sports (USA-WSWS), the Midwest Regional Show Ski Association (MRSSA), and the Waterhawks Ski Club from any responsibility for personal injury and/or damage to or loss of property or equipment during the Tournament and use of the Tournament premises.

By signing this agreement, the undersigned entering club agrees to all of the terms and conditions set forth, agrees to all NSSA rules and regulations for the tournament, and accepts the responsibility for compliance.

We have informed all members of the above agreement, and all competing members have agreed to its terms.
