Financial Responsibility
Thank you for choosing Xclusive Cranial Care. We are committed to providing exceptional cranial prosthesis services while maintaining clear financial expectations.
By signing this policy, you acknowledge that you understand and agree to the following terms.
Consultation Fees
Consultation fees are due at the time of service unless otherwise stated. Consultation fees may be applied toward your cranial prosthesis purchase if applicable.
Payment Responsibility
Patients are financially responsible for all services, products, deductibles, co-insurance, co-payments, and balances not covered by their insurance plan.
Payment is due at the time services are rendered unless prior arrangements have been made.
Insurance Benefits
As a courtesy, Xclusive Cranial Care may assist with insurance reimbursement documentation.
Insurance coverage is never guaranteed.
Coverage, reimbursement amounts, deductibles, and eligibility are determined solely by your insurance company.
The patient remains financially responsible for any unpaid balance regardless of insurance reimbursement.
Out-of-Network Insurance
If Xclusive Cranial Care is not participating with your insurance plan, payment may be collected in full at the time of service.
Upon request, we can provide documentation that you may submit to your insurance carrier for possible reimbursement.
HSA/FSA Payments
We gladly accept Health Savings Account (HSA) and Flexible Spending Account (FSA) cards when eligible under your plan.
Acceptance of an HSA/FSA card does not guarantee that your purchase qualifies under your specific benefit plan. Patients are responsible for verifying eligibility with their HSA/FSA administrator.
Custom Cranial Prosthesis Orders
Each cranial prosthesis is custom-made specifically for the patient.
Once production has begun:
Orders cannot be canceled.
Deposits are non-refundable.
Custom units cannot be returned or exchanged due to their personalized nature unless required by applicable law.
Returned Payments
A $35 fee will be charged for returned checks or declined electronic payments when applicable.
Appointment Cancellations
Please provide at least 24 hours' notice if you need to cancel or reschedule your appointment.
Late cancellations or missed appointments may result in a cancellation fee.
Refund Policy
Because cranial prostheses are customized medical devices made specifically for each patient, refunds are generally not available once fabrication has begun.
If there is a concern regarding the fit or workmanship of your prosthesis, please notify us promptly so we can evaluate the issue and determine an appropriate resolution.
