From each vendor, you will receive at least four lunches depending on the rotation schedule. Please note this semester's schedule below:
Chick-fil-A: August 20, September 17, October 15, November 12, December 17
Dominoes Pizza: September 10, October 1 & 29, December 3
Please enter your student's name and choices under each lunch menu.
If your student does not want that particular lunch, please choose "NO" and do not count in your total.
At the end, please add up the TOTAL number of lunches from each vendor and enter that in the quantity field.
Chick-fil-A: Sandwich or 8 count nuggets, chips, & cookie.
Dominoes Pizza*: Pizza, carrots, & cookies.
