We recognize that many clients seek our services for sensitive personal, relationships, family, or investigative matters. No photographs, videos, audio recordings, testimonials, case information, or identifying details will be publicly released without your express written authorization through this form. This authorization is entirely voluntary and is not a condition of receiving services from Achieve Destiny, LLC.

I understand that confidential investigative information, surveillance materials, reports, witness information, photographs, videos, records, or evidence collected during my investigation will not be publicly disclosed unless:

• I provide separate written authorization;
• disclosure is required by law or court order; or
• disclosure is otherwise permitted under the service agreement or applicable law.

Achieve Destiny will never use confidential investigative evidence for marketing or promotional purposes without my specific written consent.

I understand that I may revoke this authorization at any time by providing written notice to Achieve Destiny, LLC. Revocation will apply only to future use after written notice has been received and will not require the removal or recall of materials that have already been published, printed, distributed, or shared before the revocation became effective.

I understand that I will not receive financial compensation, royalties, commissions, or other payment for any authorized use of my image, voice, likeness, testimonial, or other approved materials unless otherwise agreed to in writing.
