The monthly golf clinic is offered to students ages 6-16. Students will receive full swing, chipping, putting and on course instruction. The clinics are offered from at various times on Monday's, Wednesday's, and Saturday’s. Clubs are available for student who do not have their own clubs. This is a month long clinic. Fees are $25 per session. Monthly fees $100 per month per student if there are 4 weeks in the month, or $125 per month if there are 5 weeks in the month.

Pricing to attend both Monday's and Wednesday's is $200 per month per student  if it is a 4 week month or $250 for 5 weeks in a month from 4:00-5:00pm. 

Please note, if you are registering for more than one student, you will need to fill out this form seperately for each student. 

 

Clinic dates are as follows:

Monday's - 7/6, 7/13, 7/20, 7/27 - 4:00-5:00pm - $100 (4 lessons)

Wednesday's - 7/1, 7/8, 7/15, 7/22, 7/29 - 4:00-5:00pm - $125 (5 lessons)

Saturday's - 7/4, 7/11, 7/18, 7/25 9:00-10:00am - $100 (4 lessons)

Release from Liabilities:

You assume your child may be at risk and danger incidental to the game of golf and the golf lesson and release the Golf Professionals, golf course, owners, employees and agents thereof from any and all liabilities resulting from such cases. Jacob Lusk (and other Golf Professionals) are running this camp at Camarillo Springs Golf Course.The golf course is not responsible for any liabilities arising out of this program nor does it know the details of the program. You do not pay the course but to the Professionals. The Golf Professionals are not responsible for children arriving early or picked up late.

Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
