Annual Back-to-School Shopping Spree Extravaganza Waitlist

Thank you for your interest in our Annual Back-to-School Shopping Spree Extravaganza.

Please note: This form is for the event waitlist only. Completing this form does not guarantee a spot at the event. If space becomes available, Voices staff will contact you using the information provided to let you know if your child has been moved from the waitlist and has a confirmed spot.

Please do not attend the event unless you have been contacted by Voices staff and notified that your child has a confirmed spot. Families who remain on the waitlist will not be able to participate in the event.

This is the only Back-to-School event that Voices will be hosting this season. We will not have backpacks or be able to provide uniforms outside of this event, so if your child needs these items, we encourage you to complete the waitlist form in the event that an opening becomes available.

If selected from the waitlist, each child will have the opportunity to shop for brand-new school clothes and pick out their own backpack filled with essential school supplies.

Date: Sunday, August 2, 2026

Time: 9:00 a.m. to 11:00 a.m.

Eligibility:
This event is open to children entering kindergarten through 12th grade with an open and active dependency case. This opportunity is not available for children younger than kindergarten or for children without an active dependency case.

To participate, the child must attend the event.

Important Information:
• This form is for the waitlist only and does not guarantee participation in the event.

• If space becomes available, Voices staff will contact you directly with additional information and confirm your child's participation.

• Please do not attend the event unless you have received confirmation from Voices staff that your child has a confirmed spot.

• Each child must attend with an adult, such as a caregiver, parent, Guardian ad Litem (GAL), CAM, DCM, case manager, or attorney. The adult must remain on-site with the child for the entire event.

• If selected from the waitlist, each child may shop for at least one complete school uniform and will also receive a backpack filled with school supplies.

• Please complete one form per child. If a question does not apply, please enter N/A.

• Please complete this form in its entirety. Some questions help our team plan appropriately and ensure a safe, organized, and positive experience should your child be offered a spot.

• Any information shared on this form will be kept confidential and used only by Voices staff for internal planning and event coordination.
