GRAVO’S GANG POLE VAULT ACADEMY 

Liability Waiver, Release of Liability, and Assumption of Risk Agreement
Please read this document carefully. By signing this document, you are waiving certain legal rights, including the right to sue Gravo’s Gang Pole Vault Academy, its coaches, and its affiliates.

 

1. Participation and Inherent Risks
I, the undersigned participant (or parent/legal guardian if the participant is under 18), acknowledge that pole vaulting is a high-risk, strenuous athletic activity. I understand that the risks include, but are not limited to:

-Tripping, falling, or missing the landing mat entirely.
-Pole breakage, equipment failure, or improper use of equipment.
-Collisions with the standards, crossbars, or the ground.
-Severe physical injury, including sprains, fractures, concussions, paralysis, or even death.

I voluntarily choose to participate (or allow my child to participate) in [Insert Club Name] practices, competitions, and events, and I expressly assume all risks associated with this sport.

 

2. Release of Liability
In consideration for being allowed to participate in [Insert Club Name] activities, I hereby release, waive, discharge, and hold harmless Gravo’s Gang Pole Vault Academy, its owners, directors, officers, coaches, employees, volunteers, and the owners/operators of the facilities where activities are held (collectively referred to as the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of negligence, personal injury, property damage, or wrongful death resulting from my participation.

 

3. Medical Representations and Authorization
I certify that the participant is in good physical health and has no medical conditions that would prevent safe participation in pole vaulting.
In the event of an injury or medical emergency, I authorize [Insert Club Name] staff to secure necessary medical treatment, including EMT transport, hospitalization, or surgery, at my own expense.

4. Rules and Safety Compliance
I agree to follow all safety rules, coaching instructions, and facility guidelines. I understand that [Insert Club Name] reserves the right to terminate my participation at any time if I fail to comply with safety protocols.

Photo/Video Release

I hereby grant the Gravo's Gang Pole Vault Academy permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web- based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the Gravo's Gang Pole Vault Academy and will not be returned.

I hereby irrevocably authorize the Gravo's Gang Pole Vault Academy to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the Gravo's Gang Pole Vault Academy from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:
