Fall 2026 Insurance Agreement

The international health insurance mandate must be fulfilled by international students for an entire academic year. Therefore, all international students automatically have an SI Hold placed on their account as needed.

To release the SI hold and be able to register during your scheduled date/time for Fall 2026, indicate how you intend to satisfy the mandate for the 2026-2027 academic year by completing/submitting this online form.

Please note: The USF Sponsored Student health insurance (SHIP) premium will automatically be assessed to your Student Self Service account when the hold is released pending the outcome of the option you choose below.

I am electing to waive/opt out of the USF Sponsored Student Health Insurance Plan (SHIP) and will provide proof of an adequate alternative health insurance policy for the 2026-2027 academic year (8/17/26-8/16/27). I understand that on July 15th I will be enrolled in the Health Insurance 101 for International Students via USF Canvas. I must successfully complete this course and quiz before I can submit proof of my alternative policy to Academic Healthplans (AHP). I will be held responsible for the annual SHIP premium until my alternative coverage is approved by both AHP and USF. I will maintain continuous coverage with no gaps.

F/J visa holder: The process for waiving the university-sponsored health insurance has changed for the 2026-2027 academic year. (1) Read the updated insurance requirements and complete the Health Insurance 101 for International Students course via Canvas (2) Ask your insurance company to complete the USF Insurance Compliance form. Upload the completed compliance form, front and back of your insurance card and a detailed policy brochure to your Care26 account with AHP. (3) New students will receive an email from Care26/AHP to activate their account approximately 3–5 days after completing the course. Returning students should log into their existing Care26 account within 3-5 days after completing the course. (4) The review process can take 3-4 weeks. Your insurance must be approved by both AHP and USF before the insurance charge is removed from your student account.
