I acknowledge that this event has a risk of a person’s physical and mental limits and carries with it the potential for death, serious injury and property loss. The risks include, but are not limited to: actions of other people including, but not limited to, participants, volunteers, spectators, coaches, event officials, event monitors and/or producers of the event; lack of hydration, weather, and/or other natural conditions. I hereby assume all of the risks of participating in this event.

This is to certify that I, as parent/guardian/team captain/lead with legal responsibility for this participant, do consent and agree to my his/her release as provided above of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to individual involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

I acknowledge that I have carefully read this "WAVIER AND RELEASE" and fully understand that it is a release of liability.  I expressly agree to release and discharge East Plano Islamic Center and all of it's affiliates, managers, members, agents, attorneys, staff, volunteers and heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of actions and I agree to voluntarliy give up or wavie any right that I otherwise have to bring a legal action against East Plano Islamic Center for Personal Injury or Property Damage.
