Dental Services Available:
- Visual Dental Screening, Fluoride Varnish Application, and Sealant Placement (a protective coating placed on molars to prevent cavities)
- A written assessment of your child’s oral health status and needs will be sent home following the screening.
Cost: There will be no cost to you.*
* CHC will bill insurance, but not you. If your child is insured, Community Health Care will bill their insurance. You will not be billed for services that are not covered. All services provided for free if your child has no dental coverage.

Common Questions...
Do I need to be there? No; your student will need a signed Permission Form turned in.
What if I don’t want my child to receive some of these services? If you choose to sign up or opt out of each dental service, please do so on the attached permission form.
I have other family and friends who are not enrolled in school. Can they be seen? Not at the screening event, but we would be happy to see them at any of our dental clinics. Call (253) 722-2151 to schedule an appointment. If they do not have insurance or have out-of-pocket costs that are difficult to cover, CHC offers services on a sliding-fee-scale determined by annual income and family size. Nobody is turned away due to inability to pay!
How do I register my student? Fill out the attached permission form and return it to their school.
