• Henry County Valor Awards - Healthcare (Frontline) Warriors Nomination Form

    Throughout Henry County, each of our communities have extraordinary individuals and organizations who are committed to improving the quality of health and healthcare of those they serve. The hard work and dedication of these heroes makes a difference in the quality of life of all residents. The 2026 Valor Awards "Healthcare (Frontline) Warriors" award is designed to honor these extraordinary individuals from healthcare and public health organizations whose efforts have made a positive impact in providing/creating services focused on closing the health disparity gap in our communities. Please complete the following nomination form for your healthcare award nominee(s). The deadline to submit your nomination is Monday, August 31, 2026. Nominations are reviewed by an independent selection committee, and award recipients will be announced at the 2026 Henry County Valor Awards Luncheon on Friday, November 13, 2026.
