📸 Photo & Video Release

Throughout the week, photos and videos may be taken during camp activities and our Friday Family Showcase. These images may be used to celebrate our performers and promote future programs.
I give Rhythm Enterprises LV LLC, Rhythm Nation Dance & Fitness Studio, Deyampert Dance, and RND Dance Company permission to photograph and/or record my child for use in:

🍎 Lunch & Camp Agreement

To help keep all performers safe and fueled throughout the day, please review the following.
I understand my performer must bring a nut-free lunch each day.
I understand my performer should bring a refillable water bottle each day.
I understand water and light snacks will be provided by the camp.
I understand that camp hours are from 9:00 AM to 4:00 PM. I agree to pick up my performer on time. A 15-minute grace period will be provided. Beginning at 4:16 PM, a $15 late fee will be charged, plus $1 per minute thereafter until my performer is picked up.
I understand my performer will participate in the Friday Family Showcase.

📝 Participation Waiver
I give permission for my child to participate in Backstage Pass: A Performing Arts Summer Experience, presented by Rhythm Nation Dance & Fitness Studio and Deyampert Dance.

I understand that participation in dance, movement, theater, games, and other physical activities involves some inherent risk of injury. I voluntarily assume these risks on behalf of my child.

I certify that my child is physically able to participate in camp activities and have disclosed any medical conditions or allergies that may require special attention.

In the event of an emergency, I authorize Rhythm Nation Dance & Fitness Studio, Deyampert Dance, and their staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I release and hold harmless Rhythm Enterprises LV LLC, Rhythm Nation Dance & Fitness Studio, Deyampert Dance, RND Dance Company, their owners, instructors, staff, volunteers, and guest artists from liability for injuries or accidents that may occur during camp participation, except where prohibited by law.
