Project Overview
The project’s primary aims are to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services during the winter period by providing targeted interventions and support toolder people in Preston who are at risk of injury due to falls. The initiative will focus on supporting frail older adults, particularly those with existing long term health conditions, and at high risk of falling. The project should focus on people living within their own homes, including those who are supported by both paid and unpaid carers, as well as considering individuals residing in a care home setting.
The project will deliver proactive outreach, the development of preventive interventions, and work in close partnership with local health and social care organisation’s, to avoid duplication add value, support individuals to access existing services where available, and address unmet need where gaps exist.
