Participants acknowledge and accept these risks when engaging in dance-related activities.Participants are responsible for assessing their own physical condition and determining whether they are fit to participate in dance classes and activities. Medical Waiver: I hereby authorize the provision of any necessary medical care for my child/children in the event of an accidental injury, illness, emergency, or non-emergency situation until I can be reached.
