- 1st - 8th: Faith Formation will be held during Release Times form the public school.
- High School will meet on Monday evenings from 7:15 pm to 8:15 pm.

Tuition: $100 per child/teen.

If you prefer, you may also pay by cash or check at the parish office. Please make checks payable to Most Holy Trinity.

Parent/Guardian Permission

I hereby give consent for my son/daughter listed on this form to participate in the 1st - 12th Faith Formation program, Release Time from Waldron Elementary & Middle School (if applicable) and related activities at Most Holy Trinity Parish, Fowler. I understand that my son/daughter will be under the supervision of the authorized parish/school personnel, and/or volunteer catechists. In consideration of my child being allowed to participate in 1st-12th Faith Formation, I agree to waive and release, and indemnify and hold harmless Most Holy Trinity Parish/School, any and all affiliated organizations, its/their employees, agents, representatives, and volunteers from any and all claims I or my son/daughter may have, excluding claims for intentional misconduct or gross negligence, arising from or relating to my child's participation.
Communication & Cancellation Acknowledgment

I acknowledge that Flocknote is the primary method of communication for Faith Formation announcements, cancellations, and schedule changes. If I have not already joined FaithFormationParent Flocknote group, I understand that I can do so by texting FORMATIONPARENT TO 84576. I also acknowledge that if Fowler Public School cancels school, all Faith Formation classes and activities are automatically canceled.
Photo Release
I give permission for pictures of my son/daughter to be taken and used on the parish bulletin, parish website and/or social media accounts without compensation. I waive my right to inspect or approve such publications, including any written copy that may be created in connection of therewith.
