Tattoos by Sys Policy

Please review the policy, provide your tattoo details, acknowledge the deposit and cancellation terms, and sign the aftercare and liability agreement.
General Policy Acknowledgment
Please send a photo copy of your ID and Tattoo Design below

Make sure the photo is easy to see.

Send over your filled out policy form within 24 hours before arriving at your appointment.

Failure to do so will result in cancellation.

All Clients must be at least 18 years of age to receive a tattoo.

No Exceptions.

Please, eat at least an hour before arriving. Intake plenty of water.

No over consumption of substance of any kind, or you will be denied that same day and forfeit your deposit.

Deposit and Cancellation Policy Acknowledgment
For tattoos that cost equal to, or more than $100, a deposit of %25 of the total cost is required. This will take away from the total cost of the tattoo.

Example: $100 = ($25 deposit) Remainder: $75

Failure to show up for your appointment or a cancellation, will result in the forfeit of your deposit. (Non-refunded.)

Aftercare Instructions and Liability Acknowledgment
Items you need to have for proper healing.
- Aquaphor or a skin friendly lubricant, fragrance free.
- Plastic wrap.
- Antiseptic Soap (Keep it clean.)
- Clean fabric or paper towel.
  Wash, Dry, Moisturize, Plastic Wrap (Week 1)
Attention!
- Do not submerge your tattoo until it is healed. No pools.
- Do not scratch, peel or itch the tattoo.
- Seek medical attention if you think you are having an allergic reaction of any kind.
- Keep the area clean. (Wash 1-2 times daily until fully healed.)
Failure to care for your tattoo is not the responsibility of the Tattooist. Damaging your tattoo by not following proper aftercare, is not the responsibility of the Tattooist.

Medical and Legal Cost for Negligence are directly the responsibility of the client.
