    This is your opportunity to look back and see just how far you've come. Please do NOT have your 'Start of PHA' in front of you. We want you to answer this based on how you feel NOW.

    Why it matters...
    Progress isn't always measured by the scales or a single symptom. Often the biggest changes are the ones we don't notice day to day—more energy, better sleep, calmer digestion, clearer thinking, less pain, improved mood, healthier skin, greater confidence, and feeling more like yourself again.

    Taking the time to compare where you are today with where you started helps you recognise just how much has changed.

    The questions...
    Please answer every question based on how you feel today. Be just as honest as you were when you completed your first PHA. Every answer helps us measure your progress, celebrate your wins, identify any remaining roadblocks, and determine the best next steps for your continued health journey.

    Your opportunity...
    This is your chance to pause and celebrate your progress. As you move through this profile, you'll probably notice changes you hadn't fully appreciated. You may also recognise areas that still need attention—and that's okay. Health isn't about perfection. It's about continual progress, and every positive step is worth celebrating.

    Please be honest...
    Not "better" honest. Real honest. If something has improved, celebrate it. If something hasn't changed as much as you'd hoped, tell us. Honest answers allow us to celebrate your achievements while giving you the best guidance for what comes next.

    Today isn't the end of your journey—it's a milestone. It's your opportunity to look back with pride, recognise how far you've come, and look ahead with confidence.

    1. We highly recommend completing your PHA on a desktop or laptop, as the written sections can be difficult to complete on a phone.
    2. At the end of the first page (section) click the SAVE button. This will prompt you to create an account (with JotForm) which then emails you a link email link to your saved form. Check junk/spam folder if the link is not in your inbox. Email comes from noreply@jotform.com 

    3. At the end of every page (section) ENSURE you click the SAVE button, then click NEXT if you are still continuing. This way you will never lose any inputted information. Your JotForm email link will take you to the place on the form where you left off.

    Please list any health products you regularly or occasionally used THROUGHOUT GUTSTRONG. This includes protein powders, collagen powders, food powders, fibre products, herbal preparations and over-the-counter products such as antacids.

    Please list any cosmetic or beauty treatments you have undergone DURING GUTSTRONG. Examples include Botox®, fillers, laser treatments, IPL, skin needling, chemical peels, cosmetic surgery and similar procedures.

    Real, lasting health isn't built by chasing the number on the scales. That keeps your focus on short-term results. Instead, we want you to focus on the daily habits that create long-term change—better food choices, improved sleep, more energy, calmer digestion, less pain, and all the other signs that your body is healing.

    Trust the process.

    If you know you'll be tempted to jump on the scales, ask a friend or family member to look after them until it's time. You might laugh... but many of our most successful members have done exactly that!
