    6th Annual Back To School Drive Event Date: July 25

    By attending and/or participating in the 6th Annual Back To School Drive, hosted by Saving Our United Leaders (S.O.U.L.), I acknowledge and agree to the following:

    Assumption of Risk & Liability Release

    I understand that participation in this event may involve certain risks, including but not limited to personal injury, property damage, or loss. I voluntarily assume all risks associated with my participation.

    I hereby release, waive, and hold harmless Saving Our United Leaders (S.O.U.L.), its founders, directors, volunteers, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to:

    ● Personal injury or illness

    ● Property loss or damage

    ● Accidents occurring during the event

    There is a photographer at this event and all photography files will be stored with the organization.

    Medical & Injury Acknowledgment

    I acknowledge that I am responsible for my own medical care and expenses in the event of injury. I certify that I am physically able to participate in this event.

    Property Responsibility

    I understand that I am solely responsible for my personal belongings and property. The organization is not responsible for any lost, stolen, or damaged items.

    Photography & Media Release

    I grant permission to Saving Our United Leaders (S.O.U.L.) to capture and use my image, likeness, voice, or participation in photographs, video recordings, or other media.

    I understand and agree that these materials may be used for:

    ● Promotional purposes

    ● Advertising and marketing

    ● Commercial use

    ● Social media and digital platforms (@soulfl, @soulNY, Saving Our United Leaders- Florida, Saving our United Leaders- NY)


    I waive any right to inspect or approve the finished media and acknowledge that I will not receive compensation for its use.
