2026 PAYHL 3-on-3 Summer Season Registration

Register as an individual player for the 3-on-3 summer season (July 31st-August 31st; dates tentative). Schedule still being made. Season will consist of 9 games and 2 guaranteed playoff games. I am trying for 4 age divisions (U9, U11, U13 & U15, but will combine if we don't reach full capacity).

Regular Players: The registration fee of $330 is due upon completion of this form.

Affiliate Players (AP): A $50 upfront fee is due upon completion of this form to cover start-up costs (jersey, t-shirt, and admin fees). If approved, APs also pay $30 per game played.

I understand that this registration is not valid until the required fee ($330 for Regular Players / $50 upfront for Affiliate Players) has been received by PAYHL.
Waiver: I acknowledge that participation in hockey involves risk of injury and I release the Prince Albert Youth Hockey League, its organizers, coaches, and volunteers from any liability for injuries or accidents that may occur on or off the ice.

Payment Terms: Payment is due at the time of registration. Registration is not complete until payment is received.

Refund Policy: A 100% refund will be issued if the league is notified in writing of the player's withdrawal on or before July 15th. A 50% refund will be issued if the league is notified of the player's withdrawal after July 15th and prior to the start of the season (July 31st). No refund will be issued once the player has started the season and subsequently withdraws from the league.

IMPORTANT: Your registration is NOT considered valid until payment has been received — the $330 season fee for Regular Players, or the $50 upfront start-up fee for Affiliate Players. Your player's spot is only confirmed once the e-transfer has arrived. Unpaid registrations may be removed to make room for other players.
