Waiver, Release of Liability & Assumption of Risk – Summit Soccer, LLC Participation in soccer training, private instruction, and camps conducted by Summit Soccer, LLC (“Summit Soccer”) involves inherent risks. Activities may occur indoors or outdoors at rented gym or field facilities owned by third parties. I understand that risks include, but are not limited to: Sprains, strains, fractures, concussions, and other head injuries; Collisions with other participants; Falls, slips, and contact with equipment; Other serious bodily injury. Injuries resulting from the actions or negligence of participants, coaches, facility owners, or Summit Soccer. Assumption of Risk I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation in Summit Soccer activities, to the fullest extent permitted by Washington law. Release of Liability To the fullest extent permitted by law, I release, waive, discharge, and agree not to sue Summit Soccer, LLC, its members, managers, employees, agents, volunteers, and affiliated facility owners from any and all claims arising out of or related to participation in the activities, including claims alleging negligence. Indemnification I agree to indemnify and hold harmless Summit Soccer, LLC from any claims, damages, liabilities, or expenses arising from my child’s participation. Medical Authorization I certify that my child is physically fit to participate. In the event of injury or medical emergency, I authorize Summit Soccer to obtain emergency medical treatment. I understand that I am responsible for all medical expenses incurred. Concussion Acknowledgment I understand that soccer involves a risk of concussion and head injury. I agree that my child will not return to participation after a suspected concussion without appropriate medical clearance consistent with Washington youth sports safety requirements. Photo & Video Release I grant Summit Soccer, LLC permission to photograph or record video of my child during activities and to use such images or recordings for promotional and instructional purposes without compensation. Governing Law & Severability This Agreement shall be governed by the laws of the State of Washington. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.
