Company Sponsored Grants Disbursement Conditions

I confirm that by registering on behalf or as a company-sponsored
participant:

1. The company is a direct employer of the participant
2. the company pays the participant's salary
3. The company provides statutory benefits such as CPF contribution,
   medical benefits, leave
4. The company's UEN provided is correct and accurate
5. Course fees must be paid by the company's GIRO/Bank Transfer/Paynow as per UEN submitted

I understand that this information submitted to SkillsFutre Singapore Agency (SSG) and the UEN will be used to verify the employer and employee relationship between the participant  and company.

If information is found to be false, I will be liable to SkillsFuture Singapore Agency (SSG) and will pay for the full course fees.

Do NOT proceed if the participant does not meet the criteria above. Please edit your course application if any of the information provided above is incorrect.

General Terms Conditions

1. Participants are only eligible for the SSG subsidy ONCE for each training course. If it is found that a participant has attended the same SSG-subsidised training course, the Company will be charged the full course fee.
2. bizSAFE 2 & 4 course only - Participants must achieve 100% course attendance to be competent and take the necessary assessment in order to be eligible for the course fee subsidy.
3. Other Courses - Participants must achieve 75% course attendance to be competent and take the necessary assessment in order to be eligible for the course fee subsidy.
4. Participants must achieve 100% passing rate for asessment to be competent.
5. The Company agrees to pay the course fee in full in the event that a participant fails to meet the required attendance rate and/or is not yet competent at the end of the course.
6. Information provided in this registration form must be true and correct. Learners Hub must be notified of any discrepancies in the information provided within 3 days upon completion of the course.
   Any errors in the Statement of Attainment or grant disbursement owing to inaccurate information given will be billed accordingly to the Company.
7. The course schedule is subject to confirmation. Learners Hub however reserves the right to cancel or re-schedule the class without prior notice.
8. Learners Hub reserves the right to conduct a survey of all participants in compliance with SSG regulations on funding.
9. All Statement of Attainments (SOA) are in e-certificate format. Using their SingPass, participants will be able to view, download and print their e-certificates by logging into www.myskillsfuture.gov.sg. Upon request, Learners Hub will assist participants in printing the e-certificates and will contact participants to either collect or mail out the hardcopy of the e-certificate/certificate once they are available for collection or delivery.
10. The nett course fee must be paid before the course commencement date.

Use Of Personal Data

1. I understand and agree that Learners Hub Pte Ltd can use my particulars above to contact me for any purpose in connection with the course I have applied for.
2. I understand that in the event I withdraw from the Course and give less than 7 days’ notice before Course commencement date or on or after Course commencement date, I shall not be eligible for any refund of Course fees already paid.
3. I agree to participate in any survey administered by Learners Hub Pte Ltd or any party for any purpose in connection with the training courses.
4. I confirm that I have read, understood and agree that I accept and shall be bound by the Company Sponsored Grants Disbursement Conditions, General Terms and Conditions and Use of Personal Data as set out in this Registration Form.

Individual Grants Disbursement Conditions

I understand that by registering as an individual:

1. Baseline Skillsfuture Grants will only be available to me if this is the first time attending the course.
2. Utilising of Skillsfuture Credit “SFC” is my own decision, and the claim will be made by me. Learners Hub is not able to and will not claim SFC on my behalf.
3. I am responsible for submitting my SFC Claim at least 1 week before the course starts

I understand that this information will be submitted to SkillsFuture Singapore Agency (SSG) and I will comply to any checks made on behalf of SSG. If any information is found to be false, I will be liable to SSG and will pay for the full course fees.

General Terms and Conditions

The Applicant consents to the collection, use and disclosure of their personal data and training records to companies that access the Ministry of Manpower (MOM)’s Training Record System (TRS). Companies using the Check Workers’ WSH Training Records eService found at the MOM website can then verify the applicant’s training records, including the following information:

1. Identification Number (Work Permit Number/FIN Number/NRIC Number)
2. Name
3. Eligibility for 4 years of Safety Orientation Course (CSOC/SSIC/MSOC) certification, if applicable
4. Course title
5. Name of Training Provider
6. Date of Assessment
7. Certificate Expiry Date
8. Result of Assessment

*If the applicant does not provide agreement, Learners Hub shall not permit the applicant to register for the course.
