(*NOTE: Sacraments after Baptism require confirmation of the Catholic/Christian Baptism, normally a baptismal certificate. For those not baptized at St. Marguerite Church, you will be required to obtain a copy of your Baptism certificate from the place of Baptism or a relative. If not available, there will need to be an affidavit by a pastor or parent certifying the Baptism took place, the date, and their confirmation it was done using a trinitarian formula.)
