If my child has physical restrictions or food allergies, I agree to discuss those with FBC VBS leaders in advance.

In the event of an emergency requiring medical treatment, I authorize representatives of First Baptist Church to secure any needed medical intervention.

As parent/guardian of the above named child, I agree to accompany said child into the church building each night and pick them up from the same location. Children will not be released to go home unless a parent/guardian is present.
