  • Over the last 2 weeks, how often has the patient been bothered by the following problems?

  • Feeling nervous, anxious, or on edge*
  • Not being able to stop or control worrying*
  • Worrying too much about different things*
  • Trouble relaxing*
  • Being so restless that it’s hard to sit still*
  • Becoming easily annoyed or irritable*
  • Feeling afraid as if something awful might happen*
  • Interpretation & Management Guidance:
    • 0 points: No anxiety disorder
    • 5-9: Mild anxiety (monitor)
    • 10-14: Moderate anxiety – possible clinically significant condition
    • 15-21: Severe anxiety – active treatment probably warranted
    Action: For scores ≥10, further assessment (including diagnostic interview and mental status examination) and/or referral to a mental health professional is recommended.

    For Panic Disorder, Social Phobia, and PTSD, a cutoff score of 8 may be used for optimal sensitivity/specificity.
  • Critical Action Reminder: Always rule out medical causes of anxiety (e.g., arrhythmias with EKG, thyroid disease with TSH) before diagnosing an anxiety disorder.
