Team Registration Waiver & Release
By submitting this form and participating in any Coast2Coast Hoops activities, I, the undersigned Team Captain/Coach/Sponsor, acknowledge and agree to the following on behalf of myself, my coaches, and all players on our roster:
1. Eligibility & Accuracy of Information
1.1 I confirm that all players listed on this team’s roster are at least 18 years of age and are participating voluntarily.
1.2 I confirm that the information provided in this registration form is true, complete, and accurate to the best of my knowledge.
1.3 I understand that Coast2Coast Hoops reserves the right to request proof of age or playing experience and to approve, deny, or remove players or teams based on eligibility and competitive fit.
2. Assumption of Risk
2.1 I understand that basketball is a physical sport that involves inherent risks, including but not limited to: falls, collisions, contact with other players, overexertion, and the risk of serious injury or death.
2.2 I understand that facilities, playing surfaces, equipment, and other conditions may present additional risks.
2.3 On behalf of myself, my team, and our players, I knowingly and voluntarily assume all such risks, both known and unknown, associated with participation in Coast2Coast Hoops.
3. Release of Liability & Indemnification
3.1 To the fullest extent permitted by law, I, on behalf of myself, my team, our players, our coaches, and our guests, hereby release, waive, and discharge Coast2Coast Hoops, its owners, directors, officers, staff, volunteers, referees, table crew, sponsors, and facility partners from any and all claims, demands, or causes of action arising out of or related to injury, illness, property damage, or loss sustained in connection with our participation in Coast2Coast Hoops activities, whether caused by negligence or otherwise.
3.2 I agree to indemnify and hold harmless Coast2Coast Hoops and its partners from any claims, damages, or expenses (including attorney’s fees) brought as a result of the actions, conduct, or participation of myself, my team, our players, coaches, or guests.
4. Medical Authorization
4.1 I certify that all players on our roster are physically fit to participate and have no condition that would make participation unsafe.
4.2 In the event of injury or medical emergency, I authorize Coast2Coast Hoops staff, referees, or facility personnel to secure medical care for any member of our team if deemed necessary, and I accept responsibility for all associated costs.
5. Rules, Conduct & Disciplinary Actions
5.1 I acknowledge that I have received, read, or have been provided access to the Coast2Coast Hoops Rules & Regulations, including game rules, timing, roster policies, and code of conduct.
5.2 I agree that our team, coaches, and players will follow all rules, respect the decisions of officials, and maintain professional behavior toward referees, staff, opponents, and spectators.
5.3 I understand that unsportsmanlike conduct, fighting, threats, harassment, or any behavior deemed dangerous or disrespectful by Coast2Coast Hoops may result in technical fouls, ejection, suspension, or permanent removal of players or the entire team without refund.
5.4 I understand that Coast2Coast Hoops has sole discretion to enforce its rules and policies.
6. Payment, Refunds & Forfeits
6.1 I understand that the team fee is due in full by the posted payment deadline and that our spot is not guaranteed until payment is received and confirmed.
6.2 I understand that all payments are non-refundable, including if our team is removed for rule violations, fails to show, or forfeits games, except as explicitly approved by Coast2Coast Hoops.
6.3 I understand that repeated forfeits, no-shows, or late arrivals may result in additional penalties or removal from the league.
7. Media Release & Promotion
7.1 I understand that Coast2Coast Hoops may photograph, film, live-stream, or otherwise record games, practices, interviews, and events.
7.2 On behalf of myself, our coaches, and our players, I grant Coast2Coast Hoops the irrevocable right to use, reproduce, edit, and publish our names, images, voices, highlights, and likenesses in any media format (including social media, live streams, promotional videos, photography, and graphics) for promotional, marketing, and league purposes, without any expectation of compensation.
7.3 I understand that all game footage, photos, and media captured by Coast2Coast Hoops or its partners is the property of Coast2Coast Hoops.
8. Communication Consent
8.1 I agree that Coast2Coast Hoops may contact me via email, phone, and text regarding schedules, updates, rule changes, payment reminders, and other league communications.
8.2 I will ensure that all information and updates from Coast2Coast Hoops are shared with my players and coaching staff.
9. Acknowledgment & Authority
9.1 I acknowledge that I have read and fully understand this Waiver & Release and am signing it freely and voluntarily.
9.2 I confirm that I am the Team Captain/GM or an authorized representative of the team and that I have the authority to bind all players and coaches on this roster to the terms of this agreement.
