ACKNOWLEDGEMENT

Completion of this form, and receipt of confirmation, is not a contract and does not guarantee reserved space. If space is available, you will receive a follow up confirmation from MHAB staff with a reservation number and room assignment. If no space is available, MHAB staff will follow up with alternate dates.

The reservation confirmation email confirms space, setup, and equipment request – specific room assignments are subject to change. All changes will be relayed to meeting organizer.

Certifications

1. Felony Conviction Notice. Texas Education Code Section 44.034 requires that a person or business entity (excluding publicly-held corporations) that enters into a contract with the District shall give advance notice to the District if the person or an owner or operator of the business entity has been convicted of a felony (“Felony Conviction Notice”). District may terminate this Contract if it determines that Organization failed to give the Felony Conviction Notice as required by the Texas Education Code Section 44.034 or misrepresented the conduct resulting in the conviction. By entering into the Contract, Organization hereby confirms compliance with Texas Education Code Section 44.034. Any change in Organization’s status regarding the required Felony Conviction Notice, occurring at any point during the term of the Contract, shall require proper written notice to the District’s superintendent of schools.

2. Certification Regarding Terrorist Organizations. Organization certifies that the Organization is not identified on the Texas Comptroller’s list of companies known to have contracts with, or provide supplies or services to, a “Foreign Terrorist Organization” as defined by the United States Secretary of State.

3. Transactions with Abortion Provider or Affiliate. Organization hereby certifies that it is not an abortion provider or affiliate of such a provider as noted in Chapter 2273 of the Texas Government Code, and the parties hereby agree that the District is relying upon Organization’s certification in this provision in entering into the Contract. Organization’s failure to accurately certify its compliance with of this provision, or any subsequent act by Organization that makes the certification in this provision invalid, will result in the Contract being voidable by the District and Organization hereby agrees to defend and indemnify the District against any action brought by the Office of the Attorney General for a violation of Texas Government Code Section 2273.003 based on Organization’s failure to comply with this provision.
