  • Photo Permission and General Release of Liability

    NOTE: THIS IS A LEGALLY BINDING AGREEMENT, READ CAREFULLY

  • I hereby authorize Women in Aviation International (“WAI”), the Metropolitan Airport Authority of Rock Island County, Illinois (“MAA”), who owns and operates the Quad Cities International Airport (“Airport”) and its Affiliates, and the Girls in Aviation Day event sponsors (collectively the “Authorized Parties”) to publish any photographs or videos taken of me or my minor child(ren) for use in any printed publications, website, social media platform, and other mediums, and for training and promotional purposes (the “Purpose”). I release the Authorized Parties and their Affiliates from any expectation of confidentiality and attest by my signature that I am the parent or legal guardian of the child(ren) listed and that I have authority to permit the Authorized Parties to use said photographs and/or video for the Purpose. I acknowledge that participation in the event is voluntary. I acknowledge that the Authorized Parties confers no rights in photos and/or video of my minor child(ren) and that neither me nor my minor child(ren) will receive financial or other compensation for use of such photo and/or video. I the undersigned, being of lawful age, do forever release, acquit, discharge, defend, hold harmless, and indemnify the Authorized Parties and their past, present or future duly elected, appointed or employed officials, directors, officers, or members of commissions, boards, heirs, successors, executors, administrators, agents, legal representatives, assigns, and subsidiaries from all manner of actions, causes of actions, debts, claims and demands for or by reason, including injury resulting in death, which may be sustained as a result of any and all property damage and/or injury claims which may or may not have been caused solely or partly by the negligence of the Authorized Parties, that may arise as a result of or in connection with participation in the Girls in Aviation Day event, including liability that may arise from any related publication or broadcast. I understand the Authorized Parties, and their Affiliates do not assume liability for property damaged, lost or stolen on the premises of the MAA while participating in the Girls in Aviation Day event. I acknowledge that this Release is also given with the express intention of binding my spouse, heirs, executors, administrators, legal representatives and assigns to the same.


    This Release contains the entire agreement between the parties and the terms of this release are contractual and will be construed in accordance with and governed by the laws of the State of Illinois, and that if any of its terms or provisions are found to be illegal or unenforceable, the validity of the remaining portions shall remain valid and enforceable.


    I further state that I have carefully read the foregoing Release and understand the contents thereof, acknowledge I have had the ability and opportunity to seek independent legal advice prior to signing this Release, and I sign the same freely and voluntarily.
