RELEASE OF LIABILITY AND ASSUMPTION OF RISK I acknowledge that participation in football activities involves inherent risks, including but not limited to sprains, fractures, concussions, spinal injuries, heat-related illnesses, permanent disability, and death. I voluntarily allow my child to participate in Dominator Sports Youth Football Camp activities and assume all risks associated with participation. I release and hold harmless Dominator Sports, its owners, directors, officers, employees, volunteers, coaches, sponsors, affiliates, facility owners, and representatives from any and all claims, liabilities, damages, expenses, losses, or causes of action arising from participation in camp activities, including claims arising from ordinary negligence. I understand that Dominator Sports does not provide medical insurance and that I am solely responsible for any medical expenses incurred. I understand that Dominator Sports is not responsible for lost, stolen, or damaged personal property, equipment, clothing, electronics, valuables, vehicles, or vehicle contents.

CONCUSSION AND HEAD INJURY ACKNOWLEDGMENT I understand that football is a contact sport and carries a risk of concussion and other head injuries. I understand that symptoms may not appear immediately and that continued participation after a concussion may result in serious injury, permanent disability, or death. I understand that camp staff may remove a participant from activities if a concussion is suspected and may require medical clearance before returning to participation.

HEAT ILLNESS AND WEATHER ACKNOWLEDGMENT I understand that participation in outdoor athletic activities may expose my child to heat, humidity, severe weather, dehydration, heat exhaustion, heat stroke, and other environmental conditions. I acknowledge that I am responsible for ensuring my child arrives properly hydrated and physically capable of participating. I understand that camp activities may be modified, delayed, suspended, or canceled due to weather or safety concerns.

CAMP RULES ACKNOWLEDGMENT I understand that participants must follow all camp rules, safety instructions, and directions from coaches and staff. I understand that fighting, bullying, harassment, disruptive behavior, unsafe conduct, or failure to follow instructions may result in immediate removal from camp without refund.

EMERGENCY MEDICAL AUTHORIZATION In the event of an emergency and if I cannot be reached, I authorize Dominator Sports personnel and emergency medical providers to secure emergency medical treatment for my child as deemed necessary. I accept full responsibility for all medical expenses associated with treatment.

By signing below, I certify that I am the parent or legal guardian of the participant listed on this registration form. I have read and understand all camp policies, acknowledgments, waivers, releases, and authorizations contained within this registration packet and voluntarily agree to all terms.
