  • This disclosure is used by Lotus Medical Career College as part of its CNA program admissions and eligibility review. Certain questions are based on the California Department of Public Health (CDPH) criminal record review process (Form CDPH 283B). Completing this form does not constitute CDPH certification or BPPE enrollment. All applicants must answer every question truthfully and completely. Providing false information may result in denial of enrollment or CNA certification.
