Program Policies & Important Information

The following agreements are required for participation in Warrior Youth Field Hockey. Please read each section carefully. By checking the required acknowledgment boxes and providing your electronic signature at the end of this form, you confirm that you have read, understand, and agree to the applicable policies, waivers, and authorizations.

PLAYER EXPECTATIONS

By participating in Warrior Youth Field Hockey, players are expected to:
- Treat teammates, coaches, officials, opponents, and spectators with respect.
- Demonstrate good sportsmanship.
- Arrive on time and prepared.
- Follow coaches' instructions.
- Encourage teammates and help create a positive team environment.
PARENT EXPECTATIONS

As a parent/guardian, I agree to:
- Support all players, coaches, officials, and volunteers.
- Model positive sideline behavior.
- Respect officials and allow coaches to coach.
- Communicate concerns respectfully.
- Help foster a positive environment for all participants.
Acknowledgement of Player and Parent Expectations
ASSUMPTION OF RISK & RELEASE OF LIABILITY

In consideration of being allowed to participate in Warrior Youth Field Hockey practices, games, clinics, camps, tournaments, private instruction, and other program activities, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in field hockey involves inherent risks, including but not limited to falls, collisions, contact with sticks or balls, sprains, fractures, concussions, permanent injury, paralysis, or death. I knowingly and voluntarily assume all risks associated with participation.

2. Compliance with Program Rules
I agree to follow all Warrior Youth Field Hockey rules, safety guidelines, and instructions from coaches, officials, and program staff. If I observe an unsafe condition or become aware of an injury or hazard, I will immediately notify a coach or program representative.

3. Release of Liability
To the fullest extent permitted by law, I release, waive, and hold harmless Warrior Youth Field Hockey, its Board of Directors, officers, coaches, volunteers, officials, sponsors, facility owners, and representatives from any claims for injury, illness, disability, death, or property damage arising from participation in program activities, including claims resulting from ordinary negligence.

4. Indemnification
I agree to indemnify and hold harmless Warrior Youth Field Hockey and its representatives from any claims or legal actions arising from my participation or my child's participation in program activities, to the fullest extent permitted by law.

5. Arbitration
Any dispute arising from participation in Warrior Youth Field Hockey activities shall be resolved through binding arbitration in the county where the program is located unless otherwise agreed by all parties. Any claim must be filed within one (1) year of the event giving rise to the claim.

MEDICAL AUTHORIZATION

In the event that my child becomes ill or injured during participation in Warrior Youth Field Hockey activities and I cannot be reached, I authorize Warrior Youth Field Hockey coaches, Board members, or designated representatives to obtain appropriate emergency medical care for my child.

I understand that every reasonable effort will be made to contact me or my designated emergency contact before treatment whenever possible.

I understand that I am financially responsible for any medical care provided.

PHOTO & MEDIA RELEASE

Throughout the season, Warrior Youth Field Hockey may take photographs and videos during practices, games, tournaments, clinics, camps, and other program events.These images may be used on our website, social media pages, newsletters, promotional materials, fundraising campaigns, and other Warrior Youth Field Hockey communications.

As the parent or legal guardian of the participant, I confirm that I have reviewed and understand the Player & Parent Agreement, Assumption of Risk & Liability Waiver, Medical Authorization, and other program policies required for participation in Warrior Youth Field Hockey.
