Due to limited staff availability during recess periods, Hovhannes Tumanyan Armenian Language School allows parents or guardians to watch or supervise their own children on the play ground or school grounds. Because school staff may not be present or able to provide full supervision, this waiver explains that participation is voluntary and at the parent’s own risk.

I, the undersigned parent or legal guardian of student mentioned above, understand and acknowledge that during recess periods, Hovhannes Tumanyan Armenian Language School may not have sufficient staff to ensure full supervision or safety of students on the playground or surrounding areas. I further understand that recess activities may involve ordinary risks of injury, including but not limited to slips, falls, collisions, or other accidents.

I acknowledge that I am voluntarily choosing to allow my child to participate in recess under my own supervision or observation, and that I may remain on-site during this time. I understand that participation is not required and that I may choose to withhold my child from recess activities if I prefer.

I accept and assume all risks of injury or harm to my child or myself arising from participation in recess activities during periods of limited or no staff supervision. I understand that Hovhannes Tumanyan Armenian Language School, its employees, volunteers, and governing bodies are not responsible for monitoring, supervising, or ensuring the safety of my child during this time.

To the fullest extent permitted by Oregon law, I release and hold harmless Hovhannes Tumanyan Armenian Language School, its officers, employees, and agents from any and all claims, demands, or causes of action for injuries, damages, or losses that may occur during my child’s recess participation while under my supervision or observation.

I have read and fully understand this waiver and assumption of risk. I am signing this agreement voluntarily and understand that it affects my legal rights.
