This is a new Approval and Workflow process for the 2026-2027 Season

This form is used by CAHA Member Organizations to request authorization to roster a Youth or Girls Tier II National Tournament Bound (NTB) team for the current USA Hockey season. Eligible divisions include Youth 14U, 16U, and 18U, and Girls 14U, 16U, and 19U.

The information submitted through this form will be reviewed by the CAHA Tier II Committee to verify that the requesting Member Organization meets all applicable CAHA Policies & Procedures for Tier II NTB participation. Submission of this form does not constitute approval. Organizations will be notified once the review process has been completed.
