There will be a 3-point contest and free-throw showdown
To help with our community literacy goals, we are requesting registrants to bring a book for a young person who may be between PreK and 8th grade to donate.
Attendees should wear comfortable clothing, tennis shoes, and may bring a basketball and a bottle of water. Make sure your name is written on ALL items.

Liability Waiver

Medical Release

In the event of a medical emergency involving my child/ward (or myself, if adult attendee), I hereby authorize the clinic hosts, their agents, employees, and any medical personnel selected by them to obtain and administer any medical care, treatment, or transportation deemed necessary. I consent to any treatment considered advisable by the attending medical professionals and grant permission for hospitalization if required. I understand that reasonable efforts will be made to contact the emergency contact listed on this form prior to treatment when possible. I agree to be financially responsible for all medical expenses incurred as a result of such treatment. I release and hold harmless the clinic, its organizers, members, volunteers, and venue from any claim, liability, or expense arising from or related to the provision of emergency medical care, except for acts of gross negligence or willful misconduct. This authorization is effective for the date(s) of the event unless revoked in writing prior to the event.

Photo/Media Release

I hereby grant Links To Basketball, its members, and coaches the right to photograph, film, and record me and/or my child named above, and to use, reproduce, publish, and distribute such photographs, video, and/or audio recordings in any media now known or later developed for promotional, educational, or other lawful purposes. I understand that these materials may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product. I understand that I will not receive any compensation for the use of these media. I release and hold harmless the clinic, its members, volunteers, and venue representatives from any liability related to the use of such photographs, videos, or recordings. This authorization is perpetual unless revoked in writing and delivered to the clinic. By signing below, I acknowledge that I am at least 18 years old or, if signing for a minor, that I am the minor’s parent or legal guardian and have the authority to grant this release.
