Coaches / Volunteers are not allowed to provide transportation for students in private vehicles UNLESS they have completed the required information below AND have prior approval from said students' family (Student Transportation Permission Slip).

Responsibilities and minimum requirements are as folows:
1. Vehicle Insurance
  Your privately-owned vehicle must have minimum insurance coverage as follows:
  - Bodily Injury: privately-owned vehicle must $50,000 / $100,000
  - Property Damage: $25,000 per accident
2. In the event of an accident, the first liability would be incurred by you and your own insurance company. Folowing this, the Juneau School District insurance would satisfy a judgment in excess of that provided by your vehicle insurance company.
3. No fees or financial charges are to be made for transporting students in a privately-owned vehicle.
4. You must have a parent/guardian-signed permission slip for each student being transported in your vehicle.
5. All passengers (driver included) must be provided and secured with seat belts at all times.
6. Your vehicle must have all required equipment in good working order. (See page 63 of the Alaska Driver's Manual)

My driver's license has not been suspended or revoked in the past five (5) years. I have never been convicted of a felony involving a minor child. I have thoroughly read and understand the responsibilities and minimum requirements involved in transporting students on field trips. I do carry the required insurance and to the best of my knowledge, my vehicle meets all Alaska vehicle requirements. I shall take all reasonable precautions in order to ensure the safety of the students. I am wiling to provide transportation for students for this field trip.
