Liability Waiver, Assumption of Risk & Registration Agreement
I understand that firearms training involves inherent risks, including the possibility of serious bodily injury or death. By voluntarily participating in this course, I assume all risks associated with firearms training and agree to follow all instructor directions, range rules, and safety procedures at all times.
I hereby release, waive, and hold harmless DFA Tactical LLC, Fully Loaded Defense LLC, and their owners, instructors, assistants, range staff, affiliates, and agents from any and all liability, claims, demands, damages, losses, or causes of action arising out of or related to my participation in this course, except in cases of gross negligence or intentional misconduct as provided by law.
I certify that I am legally eligible to possess and use firearms and ammunition under all applicable federal, state, and local laws. I understand that successful completion of this course does not guarantee the issuance of a North Carolina Concealed Carry Handgun Permit, as final approval is determined by the appropriate issuing authority.
Registration & Rescheduling Policy
- All registration fees are non-refundable.
- Students may reschedule their class one time if notice is provided at least 48 hours before the scheduled class start time.
- Rescheduling requests made less than 48 hours before the class, or failure to attend without prior notice, will result in forfeiture of the registration fee and may require a new registration to attend a future class.
Acknowledgment
By signing below, I acknowledge that I have carefully read, fully understand, and voluntarily agree to the terms and conditions outlined above. I understand that this agreement includes a release of liability, assumption of risk, and the registration policies for this course.
