By completing this form you confirm that the member is fit and healthy to take part in workshops AND public performances. However, please state information regarding you/your child’s health, mobility, behaviour and medication in order to help us meet his/her needs more fully whilst at NK. Occasionally theatre sessions may be filmed or photographed for monitoring and general press and publicity purposes, please state if you give permission for this

We are requesting one payment for NK Memberships to be paid monthly, preferably on the 1st or the 15th of the month. This should be one regular payment for the amount to cover the classes being attended. Please provide the bank reference associated the member, below.
