  • Please Note: This form is to capture interested attorneys willing to supervise students in the Supervised Practice Program of the Nevada State Bar. This is not an application for approval to supervise an applicant. If an interested applicant has applied for limited licensure pursuant to SCR 49.3, you will receive notification and a link to complete the application form for approval as a supervised practice provider at that time.
