Liability Waiver
ATHLETIC WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT There are certain risks associated with participating in AAU basketball that cannot be eliminated. These risks include: minor injuries such as scratches, cuts, bruises and strains/sprains, and major injuries such as injuries to eyes, joints, back, mouth, heart, head, paralysis and even death. Risks are also possible from the result of using training equipment or training facilities. All of these risks will be present in the activities that you and your minor will be participating in with Manitowoc United AAU basketball. Having read this agreement and having an appreciation for and an understanding of these risks, you hereby affirm that you and your minor's participation in these activities are voluntary and that you agree on behalf of yourself and your minor to all risks associated with participating in Manitowoc United AAU basketball. I am the parent/legal guardian of the above named minor. I have read this agreement and understand the terms. I understand I am surrendering legal rights on behalf of the minor and myself. I give my consent to allow my minor to participate in Manitowoc United AAU basketball activities and release and agree to this hold harmless agreement and understand Manitowoc United can't be sued, or held liable for incident to my minor while participating in this program.
