THINGS TO KNOW:

You will only have to pay the gate entry fee (if applicable) for the entire show to race with us on this date! 

 

 Below please complete only one line per driver/class. If your driver is onbly racing in one class, fill out just one box and leave the remaining boxes blank. You do not need to repeat the same driver name and class in multiple boxes.

I am aware of the dangers of letting my child attend this event on this particular date. I can not hold the company, its officers, agents, or employees liable or pursue legal action against them. This waiver absolve the company of any duty for injuries/death sustained on the premises before to, durring, or after the activity. 

By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for injury/death sustained regardless of the cause or circumstances. I will do all in my power to obey corporate staff, all safety requirements, and I will seek clarification if necessary. 

By signing this agreement, I also give consent for photographs and videos to be taken during the event where a minor may appear. 

I understand that these materials may be used for promotional or educational purposes without compensation.
