I give my permission for my child(ren) to participate in CHAMP CAMP, a Community Vacation Bible School hosted by Combat FIT Ministries, with activities conducted under the supervision of Combat FIT Ministries staff and volunteers.
I understand and acknowledge that every reasonable effort will be made to provide a safe and supervised environment. However, I voluntarily release and hold harmless Combat FIT Ministries, Bowen Combative Arts Academy (BCAA), their affiliates, directors, officers, staff, instructors, volunteers, and partners from any liability for accidental injuries, illnesses, losses, or incidents that may occur while my child is participating in camp activities, except where prohibited by law.
I understand that camp activities may include age-appropriate physical activities such as self-protection instruction, de-escalation training, basic first aid, situational awareness exercises, games, and outdoor activities.
I agree to ensure that my child is picked up promptly at the designated camp dismissal time each day. I understand that my child will only be released to me or to an individual I have authorized below.
If I am unable to pick up my child, I authorize the following adult(s) to do so on my behalf. I understand that the authorized individual must be 18 years of age or older and present a valid government-issued photo ID before my child will be released.
