By checking "I Agree" and submitting this form, you confirm that you have read, understood, and accept the terms below. This Agreement is between you (the "Creator," "you") and Luzolva LLC, a Wyoming limited liability company ("Luzolva," "we," "us").
If you do not agree to these terms, do not check the box or submit the form.
1. What This Form Is — and Isn't
Submitting this KYC form registers your interest in joining Luzolva's Creator catalog. It is not, by itself, an employment contract, a talent management agreement, an exclusivity agreement, or a guarantee of paid work. It allows us to evaluate your profile, verify your reach, and consider you for future brand and institutional campaigns.
A binding commercial relationship — including specific deliverables, exclusivity, fees, and payment terms — is only created when you and Luzolva separately sign a formal campaign or representation contract.
2. What You're Confirming Is True
By submitting this form, you represent and warrant that:
The legal name, contact information, and company/registration details you provide are accurate and that you are authorized to provide them (including on behalf of any company or agency you represent).
You own or control the social media accounts and handles you list, and the follower counts, audience demographics, and analytics screenshots/files you upload are genuine, current (within the past 30 days), and not artificially inflated (e.g., through bot followers or engagement fraud).
If you indicate "Represented by an External Agency / Talent Manager," you have the authority to register on the terms disclosed, or your manager/agency has authorized this submission.
You are at least 18 years old, or the age of majority in your country of residence. If you are a minor represented by a parent/legal guardian acting on your behalf, the parent/guardian is the one submitting this form and accepting these terms.
Providing false or materially misleading information may result in removal from our catalog and disqualification from current or future campaigns.
3. How Your Data Will Be Used
By submitting this form, you specifically authorize Luzolva to:
Analyze your public metrics (follower counts, niche, engagement, audience demographics) to evaluate fit for brand and institutional campaigns.
Share aggregated, anonymized demographic data — not your personal contact or legal identity information — with potential corporate or institutional clients for campaign matching purposes (what we call "The Pack Effect").
Store your uploaded verification files (analytics screenshots, PDFs, media kits) securely for the purpose of confirming your reach and performance.
Contact you via the email, phone, or WhatsApp number you provide regarding campaign opportunities, account verification, or administrative matters.
Your personal contact details and legal identity information are kept confidential and are not disclosed to brand or institutional clients unless and until a formal campaign contract is executed and such disclosure is necessary to perform that contract.
Full details of how we collect, use, store, and protect your data — including your rights to access, correct, or delete it — are set out in our Privacy Policy, which is incorporated into this Agreement by reference. We encourage you to read it before submitting this form.
4. About the Rates You Provide
If you select or input a baseline rate tier in Step 4, please understand:
This is a reference point only — it does not guarantee that price for any specific campaign.
After reviewing your profile, verified analytics, and the specific requirements of a campaign, Luzolva may propose a counter-offer or a different commercial structure.
Final pricing, deliverables, and terms become legally binding only once a formal campaign contract is signed between you and Luzolva LLC.
All commercial negotiations will be conducted through the contact channels you provided in this form.
5. Content Exclusions & Exclusivity
The industries you choose to exclude (e.g., Crypto/Web3, Gambling, Alcohol/Tobacco, Adult Content) will be respected as a standing instruction when we consider you for campaigns, unless you update this preference later.
If you disclose an existing exclusivity arrangement with another brand, agency, or industry, you confirm that this information is accurate and agree to notify us promptly of any changes, so we can avoid proposing conflicting campaigns.
6. No Guarantee of Selection or Income
Registering does not guarantee that you will be selected for any campaign, matched with any brand, or receive any income through Luzolva. Selection depends on client needs, campaign fit, your verified metrics, and other business factors at our discretion.
7. Your Ongoing Rights
You may, at any time:
Update or correct the information by contacting us via contact@luzolva.com
Withdraw consent for the use of your data for campaign matching (this may limit or end your eligibility for new campaigns, but does not affect any campaign contract already in force).
Request a copy of, or the deletion of, your personal data, subject to legal retention requirements (e.g., active contracts, tax records).

8. Intellectual Property & Image Rights
Submitting this form does not transfer any ownership of your content, image, or likeness to Luzolva. Any use of your name, image, content, or likeness in connection with a specific campaign will be governed exclusively by the terms of the campaign contract you separately sign, which will define the scope, duration, and compensation for such use.
9. Limitation of Relationship
Nothing in this form or Agreement creates an employment relationship, partnership, joint venture, or agency relationship between you and Luzolva, except as may be separately and expressly agreed in a signed campaign or representation contract.
10. Governing Law
This Agreement is governed by the laws of the State of Wyoming, USA, without regard to conflict-of-law principles, except where mandatory consumer or data protection laws of your country of residence (e.g., under GDPR, CCPA, POPIA, or equivalent frameworks) entitle you to additional protections, in which case those protections will apply to the extent required by law.
11. Changes to This Agreement
We may update this Agreement from time to time as our Services evolve. If you have an active registration with us, we will make reasonable efforts to notify you of material changes via the contact information you provided.
