2027 OAOP Paraoptometric of the Year Nomination Form

The 2027 Oklahoma Association of Optometric Physicians (OAOP) Annual Awards are now open for nominations! Please complete the form to the best of your knowledge.
The Oklahoma Association of Optometric Physicians (OAOP) annually honors one outstanding paraoptometric professional who demonstrates exceptional service to patients, the practice, and the profession of optometry. Certification is not required.

Eligible nominees must be employed by an OAOP member practice in Oklahoma.
Nominations remain valid for two years.

Award Includes:
- Complimentary registration to the OAOP Vision Summit
- Recognition plaque presented at the Vision Summit Awards Banquet
- $200 cash prize
