SANDWELL HAF OUTDOOR SKATEPARK SESSIONS
PARTICIPANT CONSENT, MEDICAL INFORMATION AND ACKNOWLEDGEMENT OF RISK FORM
IMPORTANT INFORMATION
These outdoor scooter and skateboard sessions are provided by Sandwell Council through the Holiday Activities and Food Programme (HAF).
Just Ramps C.I.C. has been contracted to deliver the scooter and skateboard coaching, organised activities and competitions.
The sessions take place at outdoor, council-owned skateparks. They are not being held at the Just Ramps indoor skatepark.
A completed form allows the named participant to take part at any or all of the following Just Ramps-delivered sessions:
- Wednesday 22nd July and Thursday 23rd July 2026, 4:00pm–8:00pm – Brunswick Park Skatepark, Wednesbury
- Wednesday 5th August and Thursday 6th August 2026, 4:00pm–8:00pm – Britannia Park Skatepark, Rowley Regis
- Wednesday 26th August and Thursday 27th August 2026, 4:00pm–8:00pm – Victoria Park Skatepark, Tipton
A new form will only be required if the participant’s medical information, emergency contact details or other relevant circumstances change.
Participants aged 16 or over may complete and sign this form themselves.
For participants under 16, this form must be completed and signed by a parent or legal guardian.
- Participants aged 16 or over are responsible for ensuring that any necessary medication is available and accessible during the session.
- For participants under 16, the parent or legal guardian is responsible for ensuring that any necessary medication is available and accessible during the session.
TERMS AND CONDITIONS OF PARTICIPATION
Please read the following information carefully before signing this form.
1. SCOPE OF THIS AGREEMENT
This agreement applies to the named participant’s attendance and participation in any of the Just Ramps scooter and skateboard coaching, activities and competitions delivered as part of the listed Sandwell HAF outdoor skatepark sessions.
The sessions may include:
- Scooter coaching
- Skateboard coaching
- Supervised practice
- Skills-based activities
- Games and challenges
- Organised scooter or skateboard competitions
- Use of Just Ramps hire equipment, where available
- Awarding of competition prizes
- Completion of this form does not guarantee a place in a particular coaching group, activity or competition.
Participation may be limited according to capacity, age, ability, equipment availability, weather, ground conditions and safety considerations.
2. ACKNOWLEDGEMENT OF RISK
Skateboarding, scootering and related skatepark activities involve inherent risks that cannot be completely removed, even where reasonable safety measures, coaching and supervision are provided.
These risks include, but are not limited to:
- Slips, trips and falls
- Loss of balance or control
- Collisions with other participants, skatepark users, obstacles or equipment
- Equipment moving or leaving the immediate riding area
- Cuts, bruises, sprains and fractures
- Head, neck, back or spinal injuries
- Serious injury, permanent disability or death
- Hazards arising from outdoor surfaces, changing weather and environmental conditions
By signing this form:
A participant aged 16 or over confirms that they understand the nature of the activity and voluntarily accept the ordinary and inherent risks associated with participating.
For a participant under 16, the parent or legal guardian confirms that they understand the nature of the activity and consent to the child participating with knowledge of the ordinary and inherent risks involved.
Nothing in this agreement excludes or limits liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, or any other liability that cannot legally be excluded or limited.
3. OUTDOOR AND PUBLIC SKATEPARK ENVIRONMENT
The sessions take place at outdoor, council-owned public skateparks.
Participants and, where applicable, parents or legal guardians acknowledge that:
- The skateparks may remain accessible to members of the public.
Other riders or park users may be present near the organised activity.
- Just Ramps does not own or have exclusive control over the skateparks.
Outdoor surfaces may be affected by rain, dirt, debris, damage, wear, heat or other environmental conditions.
- Sessions may be changed, delayed, restricted or cancelled where weather, surface conditions or another safety concern makes delivery unsuitable.
- Participants must remain within the areas identified by Just Ramps staff during organised coaching and competitions.
- Just Ramps staff will carry out appropriate checks and manage the areas under their control but cannot guarantee that an outdoor public environment will be completely free from risk.
4. HEALTH AND FITNESS
By signing this form, the participant aged 16 or over, or the parent or legal guardian of a participant under 16, confirms that:
- The participant is medically and physically able to take part.
- All relevant medical conditions, allergies, injuries and additional needs have been disclosed.
- Medical advice has been sought where appropriate.
- Just Ramps will be informed if the participant’s health or medical information changes before a later session.
- Just Ramps staff may refuse or stop participation where they reasonably believe continuing would present a risk to the participant or another person.
5. FIRST AID AND EMERGENCY TREATMENT
Qualified or suitably trained staff will provide appropriate first-aid assistance where required.
In the event of an accident, illness or emergency, the person signing this form authorises Just Ramps and the event organisers to:
- Provide or arrange appropriate first aid.
- Contact the named emergency contact.
- Contact the emergency services where reasonably necessary.
- Share relevant medical and emergency information with medical professionals, Sandwell Council or other appropriate persons where necessary to protect the participant’s health, safety or welfare.
- Where the participant is under 16, every reasonable effort will be made to contact their parent, legal guardian or emergency contact promptly.
Emergency action may need to be taken before a parent, legal guardian or emergency contact can be reached.
6. PROTECTIVE EQUIPMENT
Helmets and appropriate protective equipment reduce, but do not eliminate, the risk of injury.
Participants under 16 must wear a suitable helmet while taking part in organised Just Ramps coaching, activities and competitions.
Participants aged 16 or over are strongly advised to wear a suitable helmet.
Knee pads, elbow pads and wrist guards are strongly recommended for all participants.
Participants must:
Wear any protective equipment required by Just Ramps staff.
Ensure personally owned equipment fits correctly and is suitable for the activity.
Follow staff instructions concerning the correct use of protective equipment.
A limited amount of hire equipment will be available, including:
6x hire scooters
6x hire skateboards
12x helmets
12x junior one-size-fits-all pad sets
Hire equipment is subject to availability and cannot be guaranteed for every participant.
7. PARTICIPANT EQUIPMENT
Participants using their own scooter, skateboard, helmet or protective equipment are responsible for ensuring it is safe, suitable and in an appropriate condition.
For participants under 16, the parent or legal guardian is responsible for ensuring that personally owned equipment is suitable for use.
Just Ramps staff may carry out a basic visual check and may prevent the use of equipment that appears damaged, unsuitable or unsafe.
A visual check by a member of staff is not a guarantee that equipment is free from hidden defects.
Any defect, damage or concern must be reported to a member of staff immediately.
8. HIRE EQUIPMENT
Participants using Just Ramps hire equipment agree to:
- Use it only for its intended purpose.
- Follow all instructions provided by Just Ramps staff.
- Check the equipment before use and report any visible concern.
- Stop using the equipment immediately if it becomes damaged or unsafe.
- Return the equipment when requested.
- Take reasonable care of the equipment while it is being used.
- Hire equipment may be withdrawn at any time where staff have safety or behavioural concerns.
9. COACHING AND COMPETITIONS
Participants must listen to the instructors and session manager and follow all instructions.
Coaching activities and competition entry may be organised according to:
- Age
- Experience
- Ability
- Activity type
- Available space
- Equipment availability
- Safety considerations
- Just Ramps may refuse or stop a participant’s involvement in a particular exercise, coaching group or competition where staff reasonably believe it is beyond the participant’s current ability or presents an unacceptable risk.
Competition decisions, including judging, placings and prizes, will be made by the appointed staff or judges. Their decisions will be final.
10. STANDARDS OF CONDUCT
All participants must:
- Ride within their ability level.
- Maintain reasonable control of their speed and direction.
- Take turns and observe the riding lines of others.
- Respect instructors, staff, other participants and members of the public.
- Follow all staff instructions, signs, barriers and safety rules.
- Use the skatepark and equipment safely and appropriately.
- Inform a member of staff if they are unsure about an activity.
- Avoid behaviour that may endanger themselves or another person.
- Remain within the designated session area unless permitted to leave.
The following will not be tolerated:
- Bullying, harassment or discriminatory behaviour
- Fighting, threatening behaviour or deliberate physical contact
- Reckless or deliberately dangerous riding
- Deliberately damaging equipment or park facilities
- Ignoring safety instructions
- Entering restricted coaching or competition areas without permission
- Possession or use of alcohol, illegal drugs or intoxicating substances
- Failure to comply may result in the participant being removed from an activity or prevented from taking any further part in the session.
Where the participant is under 16, their parent, legal guardian or emergency contact may be required to collect them immediately.
11. PARTICIPANTS AGED 16 OR OVER
A participant aged 16 or over who signs this form confirms that:
- They consent to taking part in the listed sessions.
- They understand the nature and inherent risks of scooter and skateboard activities.
- They have disclosed all relevant medical, accessibility and support information.
- They are responsible for following the session rules and staff instructions.
- They have provided an appropriate emergency contact.
- They will inform staff immediately if they feel unwell, become injured or are unable to continue safely.
- They understand that their participation may be stopped for safety, safeguarding, welfare or behavioural reasons.
12. PARENTAL OR LEGAL GUARDIAN CONSENT FOR PARTICIPANTS UNDER 16
For a participant under 16, the parent or legal guardian confirms that:
- They give permission for the child to participate in the listed sessions.
- They understand the nature and inherent risks of scooter and skateboard activities.
- They have disclosed all relevant medical, behavioural, accessibility and support information.
- They accept responsibility for ensuring the child understands and follows the session rules.
- They will remain contactable throughout the session.
- They will arrange prompt collection if requested due to illness, injury, behaviour or another welfare concern.
13. SUPERVISION AND COLLECTION
Just Ramps has been contracted to provide scooter and skateboard coaching, activity management and safety oversight within the organised session area.
This does not make Just Ramps a general childcare provider.
For participants under 16, parental responsibility remains with their parent or legal guardian.
The supervision and collection arrangements for each HAF event must be followed.
Parents and legal guardians of participants under 16 must:
- Follow any sign-in and sign-out arrangements set by Sandwell Council, the HAF event organiser or Just Ramps.
- Provide accurate emergency contact details.
- Inform staff of any authorised collection arrangements.
- Collect the participant promptly at the end of the activity or when requested.
- Not instruct a child to leave independently unless this is permitted under the event arrangements and has been clearly communicated to staff.
- Participants aged 16 or over must follow any applicable sign-in, sign-out and event procedures.
Just Ramps is responsible for general coaching and safety oversight during the participant’s involvement in the organised scooter or skateboard activity.
Just Ramps cannot continuously monitor every participant or every other person using the wider public park.
14. RIGHT TO REFUSE OR END PARTICIPATION
Just Ramps, Sandwell Council or the event organiser may refuse or end a person’s participation where it is reasonable to do so for:
- Health and safety
- Safeguarding
- Behaviour
- Welfare
- Capacity
- Equipment availability
- Weather or surface conditions
- Operational reasons
- Anyone believed to be under the influence of alcohol or drugs will not be permitted to participate.
15. ACCIDENTS, INJURIES AND HAZARDS
Any accident, injury, near miss, damaged equipment or environmental hazard must be reported to a member of staff immediately.
Participants must stop taking part and inform an instructor if they feel unwell, experience pain or believe continuing may be unsafe.
Failure to report an incident at the time may affect the ability of Just Ramps or Sandwell Council to investigate it properly.
16. PERSONAL PROPERTY
Personal belongings and equipment are brought to the sessions at the owner’s risk.
Participants are responsible for keeping their belongings secure.
For participants under 16, their parent or legal guardian is responsible for deciding which personal belongings or equipment the child brings to the session.
Subject to any liability that cannot legally be excluded, Just Ramps accepts no responsibility for personal items that are lost, stolen, left unattended or damaged.
17. SAFEGUARDING AND INFORMATION SHARING
Just Ramps is committed to safeguarding children and young people.
Where a safeguarding or welfare concern arises, relevant information may be recorded and shared with Sandwell Council, the HAF event organiser, emergency services, medical professionals, social care or another appropriate safeguarding organisation.
Information will only be shared where reasonably necessary and in accordance with applicable safeguarding and data-protection requirements.
18. USE OF PERSONAL INFORMATION
The information provided on this form will be used for:
- Administering participation in the sessions
- Managing health and safety
- Responding to medical emergencies
- Safeguarding participants
- Contacting participants, parents, legal guardians or emergency contacts
- Recording and investigating accidents or incidents
- Meeting contractual, insurance or legal requirements
- Relevant information may be shared with Sandwell Council, the HAF event organiser, medical professionals, emergency services, insurers or safeguarding authorities where reasonably necessary.
Medical and emergency information will only be available to personnel who need it to deliver the activity safely or respond to an incident.
19. CHANGES TO INFORMATION
The participant aged 16 or over, or the parent or legal guardian of a participant under 16, must inform Just Ramps before attending a later session if any of the following changes:
- Medical information
- Medication
- Allergies
- Emergency contact details
- Accessibility or additional support requirements
- Collection arrangements
- Any other information that may affect safe participation
DECLARATION
This form must be signed by:
The participant, where they are aged 16 or over; or
A parent or legal guardian, where the participant is under 16.
The participant may also sign alongside their parent or legal guardian where they are under 16 and are able to understand the agreement.
By signing below, I confirm that:

☐ I consent to the named participant taking part in scooter and skateboard coaching, activities and competitions delivered by Just Ramps at any of the listed Sandwell HAF sessions.
☐ I understand that the sessions take place at outdoor, council-owned public skateparks and not at the Just Ramps indoor skatepark.
☐ I understand that members of the public and other park users may be present.
☐ I agree that the participant will follow instructions from Just Ramps staff, Sandwell Council staff and event organisers.
☐ I consent to appropriate first aid and emergency action being taken where reasonably necessary.
☐ I understand that Just Ramps must be informed if any information on this form changes.
☐ I understand that participation may be refused or stopped for safety, safeguarding, welfare or behavioural reasons.
