1. Activities
I give permission for my child(ren) to participate in all Keyport Hooks and Hope activities, including but not limited to pier and shore fishing, small boat outings, private charters on the Keyport Princess, and any other events or activities organized by the group.

2. Assumption of Risk
I understand and acknowledge that these activities involve inherent and significant risks, including but not limited to drowning, falling, cuts, slips, boating accidents, collisions, weather-related hazards, and other water-related dangers. I voluntarily assume all risks, known and unknown, on behalf of my child(ren).

3. Release of Liability
I, on behalf of myself and my child(ren), hereby release and forever discharge Keyport Hooks and Hope, its founder Bill, all officers, directors, members, volunteers, the Keyport Princess and its crew, and any other affiliated persons or entities from any and all claims, demands, actions, or causes of action arising out of or related to my child(ren)’s participation in any activities. This release includes claims for personal injury, death, property damage, or any other loss, whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. Medical Authorization
In the event of an emergency, I authorize Keyport Hooks and Hope leaders, volunteers, or the Keyport Princess crew to seek emergency medical treatment for my child(ren). I agree to be financially responsible for all medical expenses incurred.

5. Photo/Video Release
I grant Keyport Hooks and Hope permission to use photographs and video recordings of my child(ren) for promotional, marketing, and social media purposes, including but not limited to the group’s website, Facebook, and other platforms.

6. Rules
I agree that my child(ren) will follow all safety rules, instructions, and guidelines provided by group leaders and the Keyport Princess crew. I understand that failure to follow these rules may result in removal from activities.
