This Monthly Reset Membership Agreement is entered into between Verbena Holistic Center and the "Member". 

The Monthly Reset Membership includes one 60-minute Verbena Signature Massage or Specialty Massage per monthly billing cycle.

Membership services are available once per monthly billing window, based on the member's activation date. The member may book their monthly massage on or after the same calendar day each month going forward.

If the member does not book or redeem their monthly service during the applicable billing cycle, the unused service will convert to a credit equal to the Monthly Reset Membership price. This credit may be applied toward a full-priced service and will expire 30 days after the end of that billing cycle. Reset Monthly Memberships are exclusively limited based on capacity to ensure a high-quality experience for our valued members.

Monthly Reset Members receive 10% off additional holistic services booked outside of their included monthly massage. Bundles, sales, promotions, and special offers cannot be combined with any other type of service or offer, including the membership discount.

Members may upgrade their regular monthly massage at any time and receive 10% off the regular price difference of the selected upgrade.

Members receive 10% off ALL full-priced retail items in Verbena's retail selection.

A three-month initial commitment with automatic credit card renewal is required. By purchasing the Monthly Reset Membership, the member agrees to an initial three-month commitment. The monthly membership rate of $110.00 (one hundred and ten dollars) will be charged to the credit card on file every month beginning on the member's activation date.

After the initial three-month commitment is complete, the membership will continue to automatically renew every month unless the member chooses to suspend or terminate the membership. Suspensions are available upon request and based on availability for up to 4 billing cycles once per year.

A suspension or termination request must be submitted in writing to Verbena Holistic Center at least 5 business days prior to the member's next monthly billing date.

By signing and dating below, I acknowledge and agree to the terms of the Verbena Monthly Reset Membership.
