PARTICIPANT WAIVER
By registering a team for the Play Like Ray Challenge, the Team Captain confirms that:

• Every player listed on this roster has agreed to participate voluntarily.

• Every player understands that hockey is a physical sport involving inherent risks including serious injury.

• Every player agrees to follow all tournament rules and arena policies.

• Every player releases and holds harmless the Play Like Ray Challenge, its organizers, volunteers, sponsors, officials, venues, KidSport Richmond, the Forward Foundation, and affiliated organizations from liability arising from participation except where prohibited by law.

• Every player confirms they are medically fit to participate.

• Every player understands photographs and video may be used for future promotional purposes.

The Team Captain certifies they have authority to register this team and acknowledge this waiver on behalf of every participant listed.
