Thank you for choosing White Stone Volunteer Fire Department, and your willingness to serve your community. You are making an application for Active Fire Fighter Membership. You must be at least 21 years of age to apply for membership. You will need to complete the full application, with all requested information. Return it to the President prior to the first Wednesday of the month in which you would like to apply. The application will be read and tabled for thirty days. After the reading you must attend the third Wednesday monthly training @ 19:30 to meet some of the members and get familiar with our building. Your application will be read again and voted on at the next monthly business meeting. You will need at least 2/3 majority of the voting members vote to start your one-year probationary membership. At this point, you will receive full PPE, including Helmet, coat, bunkers, boots, gloves, hood, response alert device, and access to the building. After your one-year probation, you will be voted on, again needed 2/3 majority of the voting members vote to be accepted into full membership.

As a member of White Stone Volunteer Fire Department, your help is needed with the day-to-day field operations on the incident scene. Also, WSVFD holds 12 monthly business meetings on every first Wednesday @ 19:30 of each month, as well as 11 monthly Trainings on the third Wednesday @ 19:30 of each month. In order to maintain your membership, you must attend a least six of twelve meetings and you must attend five of eleven monthly trainings. Attending as many of these as possible will make you a more valuable member. If your current primary work schedule interferes and is conflicting with meeting and training dates, you must notify the President, or the Vice President in reference to an absence to a business meeting. You must notify the Chief or the Training Officer in reference to an absence to a monthly training, PRIOR to the scheduled start time of either. WSVFD conducts monthly cleaning details on every second Wednesday, as well as new member training every fourth Wednesday. Times and location for these new member trainings are subject to change and are not required to maintain membership. They are, however, highly recommended.

All new members SHALL be required to complete Fire Fighter I, or the Chief's approved comparable in-house class and an Emergency Vehicle Operations Class within two years of the start of Probationary Membership in the Department.

White Stone Volunteer Fire Department is a non-profit organization, and even though it receives funding from our local county annually, the department still has to fund raise to continue to budget its daily and monthly costs of operation. WSVFD holds numerous fundraising events that are MANDATORY for all active members, requiring participation by you to the expectation of the event chairperson/scheduler. Work does not constitute as an excused absence from any fundraiser. In the event that you are unable to be at a particular event, you must have someone cover your shift/post. This is YOUR responsibility, NOT the Chairperson or Coordinator's responsibility. It would be preferred that you find someone in the WSVFD membership, WSVFD Associate membership, or from Kilmarnock, or Upper Lancaster Fire Departments. Family and friends are permissible. If you miss two Mandatory fundraisers unexcused in ONE year, you are subject to removal from membership. This year is not a calendar year, but rather a year from event to event.

I hereby apply for membership in the White Stone Volunteer Fire Department, Inc. and attest that the information I have given is both true and complete. I understand that, should the information be false, incorrect, or incomplete, it will be cause for immediate discharge from the department.

(1) I understand that I must complete the required training within the specified time as required by the department.

(2) I understand that I must work my assigned post at all fundraisers conducted by the department. Failure to work any of these fundraisers will result in an unexcused absence. Two such absences within a 12-month period will result in my dismissal from the department.

I hereby authorize the White Stone Volunteer Fire Department, Inc. to seek information from any person named in this application and/or any other person deemed necessary or desirable to contact in order to process this application. I hold the White Stone Volunteer Fire Department, Inc., its authorized personnel, and any other persons seeking or providing information relative to this application free from all liability in connection therewith.

I understand that the White Stone Volunteer Fire Department, Inc. is not bound to accept this application according to the date recorded hereon.

I agree to attach a copy of my driving record and criminal history to this application. The driving record and criminal history will be reviewed by the President only. Failure to furnish either of these records will result in the application not being processed.

I also agree to submit to periodic drug testing if deemed necessary in accordance with the department's Standard Operating Procedures (S.O.P.s).
