Parent/Guardian Acknowledgment, Liability Waiver

In consideration of my child being permitted to participate in the Florida Connect Academy BuildHER Pizza Night on Tuesday, August 11, 2026, from 5:00 PM to 7:00 PM, including all related activities associated with the event, I, the undersigned parent or legal guardian, acknowledge, appreciate, and agree to the following:

I understand that while BuildHER Pizza Night is a social and networking event, participation may involve certain inherent risks, including but not limited to slips, falls, accidental injuries, food-related incidents, or other unforeseen circumstances. I voluntarily assume all risks, both known and unknown, associated with my child's participation in this event.

I certify that my child is physically able to participate in the activities planned for the event and agree that my child will comply with all event rules, instructions, and expectations established by Florida Connect Academy, event staff, volunteers, and the event venue.

On behalf of myself, my child, my heirs, assigns, personal representatives, and next of kin, I hereby release, waive, indemnify, and hold harmless Florida Connect Academy, its employees, officers, directors, volunteers, representatives, sponsors, partners, the event venue Hangar 38, and all affiliated organizations from any and all claims, demands, causes of action, liability, damages, losses, or expenses arising from my child's participation in this event, including those resulting from ordinary negligence, to the fullest extent permitted by law.

Photo & Media Release
I understand that photographs and video recordings may be taken during BuildHER Pizza Night. By signing below, I grant permission to Florida Connect Academy to photograph, record, and use images, video, and audio of my child for educational, promotional, marketing, social media, website, print, and other communication purposes without further notice or compensation.

I understand that these materials may be used to promote future BuildHER and Florida Connect Academy programs. I release Florida Connect Academy, its employees, volunteers, and partners from any liability arising from the use of such photographs, videos, or recordings.
